In-Hospital Mortality with NOAC-Related ICH vs. Warfarin-Related ICH

Observational data show that for patients with intracerebral hemorrhage, prior non–vitamin K antagonist oral anticoagulant use was associated with lower in-hospital mortality than prior warfarin use.

One of the principal advantages of non–vitamin K antagonist oral anticoagulants (NOACs) over warfarin is the lower rate of intracerebral hemorrhages (ICHs) observed in pivotal randomized trials. But because the absolute number of ICHs in these studies was relatively low, and because specific reversal agents for NOAC-related ICHs have only recently become available, it was unclear whether this advantage would be outweighed by NOAC-related ICHs that were more severe or that produced worse outcomes.
