Among the myriad controversies regarding the management of patients undergoing radical prostatectomy for locally advanced prostate cancer is the optimal timing of radiotherapy. Although numerous randomized trials provide evidence (mainly progression-free survival data) supporting the use of adjuvant radiotherapy (ART), its use is limited, given concerns about urinary incontinence and erectile dysfunction, whereas the use of surveillance followed by early salvage radiotherapy (ESRT) is more widespread.
