To prevent venous thromboembolism (VTE) after total hip arthroplasty (THA) and total knee arthroplasty (TKA), guidelines from the American College of Chest Physicians endorse several alternatives: low-molecular-weight heparin, fondaparinux, direct-acting oral anticoagulants (apixaban [Eliquis], dabigatran [Pradaxa], or rivaroxaban [Xarelto]), low-dose unfractionated heparin, warfarin, or aspirin (Chest 2012; 141[2 Suppl]:e278S). In this Canadian double-blind, randomized, noninferiority trial that involved 3424 participants, two of these agents — aspirin and rivaroxaban — were compared directly. About half the participants underwent THA, and half underwent TKA.
