The Bruton tyrosine kinase inhibitor ibrutinib and the BCL2 inhibitor venetoclax have single-agent activity in relapsed mantle cell lymphoma (MCL).
To test the combinatorial use of these two agents, investigators conducted an industry-sponsored, single-group, phase II trial involving 24 patients with relapsed or refractory MCL. One patient had not received prior MCL therapy; half had p53 mutation, an adverse prognostic marker. Patients received ibrutinib (560 mg daily for 4 weeks) followed by venetoclax (ramped up to 400 mg daily, to mitigate tumor lysis syndrome, a known complication of the drug).
