The transition from pediatric to adult healthcare presents multiple challenges for patients with special healthcare needs. Care coordination recommendations for transitioning care have been made by professional organizations (NEJM JW Pediatr Adolesc Med Oct 2011 and Pediatrics 2011; 128:182), but their implementation has not been broadly evaluated.
