The Affordable Care Act specifies 30 days as the window for potentially preventable hospital readmissions; however, little is known about preventability of readmissions over time. In this prospective study, conducted in 10 U.S. academic medical centers, researchers evaluated >800 general medicine patients and compared readmissions within 7 days after discharge (early readmissions) with readmissions between 8 and 30 days after discharge (late readmissions). To assess preventability, two physicians at each site reviewed all available data (including surveys of patients and their hospital-based and primary care physicians) and jointly provided preventability ratings based on the Ideal Transition in Care framework (J Hosp Med 2013; 8:102).
