De-escalation and Discontinuation of Antibiotics for Febrile Neutropenia?

Applying an antibiotic stewardship program to high-risk febrile neutropenia patients resulted in reduced use of broad-spectrum antibiotics without compromising outcomes.

Guidelines on antibiotic stewardship in hematology have been published by the Infectious Diseases Society of America (IDSA; Clin Inf Dis 2011; 52:427) and the European Conference on Infections in Leukemia (ECIL) group (Haematologica 2013; 98:1826). Both publications encourage de-escalation and discontinuation of antibiotics started empirically for febrile neutropenia (FN) for responding patients, even if neutropenia persists. However, many patients are continued on the empirically started antimicrobial regimen until neutrophil recovery, a practice that contributes to excessive antimicrobial use in hematology units.
