Controversy continues about n-3 (omega-3) fatty acid supplements for the prevention of atherosclerotic heart disease. Evidence of benefit in trials is quite thin, but sales continue, seemingly unabated by the data, and expert groups continue to promote the supplements. The ASCEND trial (which also included a trial of aspirin, reported separately; NEJM JW Cardiol Aug 26 2018; [e-pub]) evaluated the efficacy and safety of daily supplementation with n-3 fatty acids on primary prevention for people with diabetes (NCT00135226). The investigators randomized 15,480 patients with diabetes, but without known cardiovascular disease, to 1-g capsules containing either n-3 fatty acids or placebo (olive oil).
