Endovascular thrombectomy (EVT) has been found to be useful for patients with ischemic stroke in two time windows: within 6 hours and from 6 to 24 hours. Penumbral imaging attempts to delineate the amount of infarcted core tissue from the amount of tissue at risk due to hypoperfusion; the difference is referred to as the area of “mismatch” or penumbra and indicates the volume that might be salvageable. Penumbral imaging plays a key role in the second time window; to investigate its role in the first window, investigators pooled data on 1764 patients (mean age, 66; 53% men) from seven trials of EVT that included baseline penumbral imaging with perfusion computed tomography [CT] or diffusion MRI.
