In vivo prediction of the histology of diminutive (≤5 mm) colorectal polyps detected at colonoscopy can be achieved using currently available endoscopic technologies. According to the American Society of Gastrointestinal Endoscopy's Preservation and Incorporation of Valuable Endoscopic Innovations (PIVI) recommendations, the following performance thresholds for optical diagnosis must be met for polyps characterized with high confidence: For resect-discard, ≥90% agreement in surveillance intervals, and for diagnose-leave, ≥90% negative-predictive value (NPV) for neoplastic polyps in the rectum and sigmoid. It is not known whether these thresholds can be met for optical diagnosis of small (6–9 mm) polyps.
