Current guidelines do not recommend antiplatelet or anticoagulant medication adjustment prior to fecal immunochemical test (FIT) screening, but there are few data regarding the newer class of direct-acting oral anticoagulants (DOACs), which includes dabigatran, rivaroxaban, apixaban and edoxaban.
Investigators in Norway conducted a secondary analysis within a large, ongoing colorectal cancer (CRC) screening trial comparing sigmoidoscopy with biennial FIT (positivity cutoff: 15 µg hemoglobin/g feces). The analysis included patients with a positive FIT, including approximately 1000 patients on aspirin, 150 on warfarin, and 200 on a DOAC, all matched with nonusers of their respective drug by age, sex, medical center, and FIT round.
