Fecal immunochemical tests (FIT) provide a noninvasive screening option for diagnosing colorectal cancer (CRC). Fewer than two thirds of eligible U.S. adults (age range, 50–75) have undergone recommended CRC screening, and FIT elicits higher screening participation rates than do more-invasive CRC screening methods. Investigators performed a meta-analysis of 31 studies in which sensitivity and specificity of one-time FIT screening, relative to colonoscopy, were determined in >120,000 participants.
FIT screening performance for diagnosing CRC varied, based on testing cutoffs used:
10 µg/g cutoff: sensitivity, 91%; specificity, 90%; positive likelihood ratio, 9.2; negative likelihood ratio, 0.1
20 µg/g cutoff: sensitivity, 75%; specificity, 95%; …
