In men with localized, favorable-prognosis prostate cancer detected by prostate-specific antigen (PSA) screening, active surveillance (AS) — rather than definitive treatment with prostatectomy or radiotherapy — is employed increasingly. However, younger men might be more likely to choose (or be directed toward) definitive treatment because they generally have fewer comorbidities and longer life expectancies than do their older counterparts. In this observational two-center study from Boston and Toronto, researchers examined outcomes with AS in roughly 2100 men; about 20% were younger than 60. Median ages of the younger and older cohorts were 55 and 69, respectively. Median follow-up was 6 years.
