Serrated polyposis syndrome (SPS) is the most common polyposis syndrome. The World Health Organization diagnostic criteria are: I) at least five serrated polyps proximal to the sigmoid, with ≥2 being ≥10 mm; II) any serrated polyps proximal to the sigmoid in an individual who has a first-degree relative with SPS, or; III) ≥20 serrated polyps of any size, distributed throughout the colon. Guidelines recommend intensive surveillance, but patients with SPS do not have homogeneous risk for developing advanced neoplasia (AN).
