Recent years have seen tremendous interest in endoscopic approaches to gallbladder drainage in patients with cholecystitis who are not candidates for cholecystectomy due to comorbidities or poor surgical candidacy. Endoscopic approaches can save patients from undergoing placement of a percutaneous cholecystostomy tube, which is often a permanent implant. Transluminal approaches involve the endoscopic ultrasound (EUS)–guided placement of a lumen-apposing metal stent (LAMS) into the gallbladder while transpapillary approaches involve placement of a double-pigtail stent (with one pigtail in the gallbladder and the other in the duodenum) during endoscopic retrograde cholangiopancreatography (ERCP).
