Takotsubo cardiomyopathy (TC), also known as stress cardiomyopathy, can mimic features of acute myocardial infarction (MI), including hemodynamic instability and shock. But how does shock associated with TC compare with that associated with MI?
In a large database of U.S. inpatient admissions with shock caused by MI or TC, only 1.2% of shock cases were attributed to TC. In unadjusted analysis, patients with TC were more likely to be younger, female, and white, and to have fewer comorbidities than patients with MI. Respiratory failure was more common in TC shock, whereas cardiac arrest and kidney injury were more common in MI shock. In-hospital mortality was lower among TC shock patients than among MI shock patients (15% vs. 37%).
