A goal of managing febrile urinary tract infections (UTIs) in children is to avoid renal scarring. To determine when scarring occurs in this setting, investigators conducted a post hoc analysis of 345 children with febrile UTIs (mean age, 19.4 months; 89% girls; 64% with vesicoureteral reflux) who were involved in the prospective, multicenter RIVUR and CUTIE studies (N Engl J Med 2014; 370:2367 and Pediatrics 2015; 136:e13, respectively); children from these studies who had no preexisting renal scars or antibiotic prophylaxis were included in the current analysis. The researchers assessed technetium 99m dimercaptosuccinic acid kidney scans of the children obtained within 16 weeks of each febrile UTI episode and after 2 years.
