Spironolactone has become a favored add-on drug choice for treating patients with resistant hypertension. However, spironolactone causes hyperkalemia — and thus is relatively contraindicated — in patients with renal dysfunction. In an industry-sponsored study, researchers examined whether patiromer (Veltassa) could mitigate this problem. Patiromer is a nonabsorbed, cation-exchange polymer that increases fecal potassium excretion and is FDA-approved to manage non–life-threatening hyperkalemia.
