Macrolides for Community-Acquired Pneumonia, Revisited by Artificial Intelligence

Macrolides appear to benefit only patients without chronic respiratory disease or cardiovascular comorbidities and those with chronic respiratory disease and high leukocytes in respiratory secretions.

Considerable controversy surrounds whether macrolides in addition to ß-lactams are beneficial in the treatment of community-acquired pneumonia (CAP). Although macrolides expand the spectrum of an antibiotic regimen to atypical pathogens and are supposed to have immunomodulatory properties, they are also associated with potential cardiovascular toxicity and potential drug interactions.

To find out which patients might benefit most from macrolide treatment, researchers used the European CAPNETZ database, which contains detailed data on the course and treatment of 11,818 adult patients with CAP treated in 25 centers between October 2002 and June 2017.
