Serum testosterone slowly declines in most men after age 30, but the risks and benefits of repletion in older men with low levels have not been well defined. ACP researchers reviewed 38 studies of testosterone replacement, with a mean participant age of 66 and follow-up ranging from 6 months to 3 years. Definitions of “low” testosterone levels varied considerably, but baseline testosterone levels were <300 ng/dL in most studies. The analysis showed that older men might sustain small improvements in sexual function with testosterone treatment but are unlikely to experience improvement in energy, mood, or cognition.
