Aspirin Dose Accompanying Immunoglobulin Therapy for Kawasaki Disease

Low-dose aspirin was not associated with higher rates of recrudescence of fever or development of carotid aneurysms compared with high-dose aspirin.

Standard therapy for Kawasaki disease (KD) includes intravenous immunoglobulin therapy plus oral aspirin, although the most effective and safe dose of aspirin is uncertain. To explore this issue, researchers conducted a retrospective review of 260 children (median age, 2.5 years) with a new diagnosis of KD. They compared the recurrence of fever following therapy with immunoglobulin and either low-dose aspirin (<10 mg/kg/d; median dose, 4 mg/kg/d) or high-dose aspirin (≥10 mg/kg/d; median dose, 76 mg/kg/d) given within 10 days of onset of symptoms. Results were as follows:

Twenty-five percent of the cohort experienced recurrent fever requiring retreatment; this rate was similar with low-dose aspirin (27%) or high-dose aspirin (22%).
