Janus kinase (JAK) inhibition is a mechanism to treat chronic immune conditions. The oral JAK1 inhibitor upadacitinib was recently tested in an industry-funded, multinational, phase II trial for induction and maintenance of remission in patients with moderate-to-severe Crohn disease. All were refractory to immunosuppressants and tumor necrosis factor antagonists.
In the induction phase, investigators randomized 220 patients to receive various doses of oral upadacitinib (3, 6, 12, or 24 mg twice daily or 24 mg once daily) or placebo; the co-primary endpoint was clinical and endoscopic remission at week 16. Eighty-two percent of screen failures were because patients' disease scores were too low. Patients were followed out to week 52.
