Janus kinases (JAKs) are intracellular signaling molecules that mediate the effects of numerous cytokines. Several JAK inhibitors currently are in development, and two are approved already for managing rheumatoid arthritis and myeloproliferative disease. Ruxolitinib (Jakafi; FDA approved for myelofibrosis only) is a JAK1/JAK2 inhibitor that is being studied for atopic dermatitis.
In a phase 2, industry-funded study, 307 adults with mild-to-moderate eczema were randomized to topical ruxolitinib, triamcinolone acetonide cream, or placebo. After 4 weeks, the Eczema Area and Severity Index score improved significantly more in ruxolitinib recipients (72% eczema reduction) than in placebo recipients (16% reduction); improvement with triamcinolone …
