The standard of care for most patients with rheumatoid arthritis (RA) and other inflammatory arthritides includes low-dose methotrexate (LD-MTX) and daily folic acid. The safety profile of methotrexate has been determined largely by meta-analyses of observational data and not from placebo-controlled trials. The most serious adverse events associated with methotrexate include cirrhosis and pulmonary effects, but many others, including hematologic malignancies, nausea, diarrhea, headache, and alopecia, have been reported.
