A Novel Neuroprotection Medication in Acute Ischemic Stroke

Nerinetide did not improve clinical outcomes when used along with alteplase. Could a signal of benefit in those without alteplase treatment be real?

The novel compound nerinetide is designed to interfere with postsynaptic density protein 95 (PSD-95). PSD-95 protein-protein interactions are believed to contribute to excitotoxic cell death in cerebral ischemia. Nerinetide has been tested in animal models and for safety in patients undergoing interventional cerebrovascular procedures and has been identified as a promising neuroprotective agent.

These authors conducted a multicenter, partially manufacturer-funded trial in 1105 adults (mean age, 71 years; 50% women; median NIH Stroke Scale score, 17) with acute ischemic stroke who were enrolled within 12 hours (mean, 187 minutes) of stroke onset and could be treated with endovascular thrombectomy (EVT).
