Preexposure prophylaxis (PrEP) with emtricitabine/tenofovir disoproxil fumarate (FTC/TDF) has reduced HIV acquisition in many countries; indeed, PrEP expansion is a pillar of the U.S. initiative to end the HIV epidemic (https://www.hrsa.gov/ending-hiv-epidemic). In 2019, emtricitabine/tenofovir alafenamide (FTC/TAF) was approved for PrEP in men who have sex with men and transgender women. TAF has smaller effects on markers of bone and kidney disease than TDF and has been promoted as a safer version of tenofovir. Because generic FTC/TDF is expected in late 2020 or early 2021 and is likely to decrease costs of this formulation, investigators have conducted a cost-effectiveness analysis comparing branded FTC/TAF with generic FTC/TDF.
