Monoclonal antibodies directed against proprotein convertase subtilisin–kexin type 9 (PCSK9) can reduce LDL cholesterol and lower cardiovascular risk when added to statin therapy. However, these drugs require injections every few weeks. Inclisiran, a non–FDA-approved drug, is a small interfering RNA that blocks hepatic PCSK9 synthesis and is effective with administration at much longer intervals. ORION-9, a manufacturer-sponsored, randomized phase 3 trial (NCT03397121), evaluated the effect of inclisiran in people with heterozygous familial hypercholesterolemia taking the maximum dose of statin.
