Ovarian stimulation followed by intrauterine insemination (IUI) increases the chances of multiple gestation. To examine this risk, investigators conducted a retrospective study of 24,649 patients undergoing 50,473 IUI cycles at a single fertility center. Ovarian stimulation was conducted with agents including clomiphene citrate, letrozole, and gonadotropins.
In women younger than 38, raising the number of mature follicles at IUI from 1 to 5 was associated with an increase in clinical pregnancy rates from 14.6% to 21.9% (adjusted odds ratio, 1.6), primarily due to an increase in multiple gestations per cycle from 0.6% to 6.5% (aOR, 9.9).
