Risk-reducing bilateral salpingo-oophorectomy (rrBSO) represents the standard of care for preventing ovarian cancer in women with BRCA germline mutations. This surgery is offered to such women when they have completed childbearing, by age 35 to 40 (BRCA1 carriers; lifetime risk of ovarian cancer, 39%–46%), or by age 40 to 45 (BRCA2; risk, 10%–27%). However, some carriers are reluctant to undergo BSO because of concerns about adverse effects (e.g., hot flashes, osteoporosis, cardiovascular or neurodegenerative disease) caused by early surgical menopause in the absence of hormone therapy.
