Plasma neurofilament light chain (NfL) levels are markers of active axonal injury and neuronal degeneration and are a potential noninvasive biomarker for dementia. These researchers analyzed cross-sectional and longitudinal plasma NfL levels and cognitive performance between carriers and noncarriers of the autosomal dominant presenilin 1 E280A mutation for Alzheimer disease (AD). Of about 1070 participants in each group at baseline, 240 to 260 participants had a mean 6 years of longitudinal data. The carrier group included cognitively unimpaired and impaired participants. In previous studies, carriers' mean age of conversion to mild cognitive impairment (MCI) was 44 years and to dementia was 49 years.
