Predicting which patients with Staphylococcus aureus bacteremia (SAB) will develop infective endocarditis (IE) can be difficult. Transesophageal echocardiography (TEE) has been recommended for all patients with SAB, but this procedure is invasive, costly, and not always available. To better assess who with SAB is at risk for IE, a scoring system called PREDICT has been tested retrospectively (NEJM JW Infect Dis Aug 2015 and Clin Infect Dis 2015; 61:18). In this model, points are allocated for how SAB was acquired, whether a cardiac device was implanted, and whether bacteremia was present ≥72 hours.
