Colorectal cancer (CRC) disproportionately affects Black people, leading several professional societies to recommend starting screening before age 50. However, the uptake and yield of early screening in Black individuals are unclear. In this prospective study, investigators at a large U.S. integrated healthcare organization mailed one-time fecal immunochemical test (FIT) kits to Black members aged 45 to 50 years and to comparison groups of previously unscreened Black, white, Hispanic, and Asian/Pacific Islander members aged 51 to 56 years. Among over 10,000 Black individuals aged 45 to 50 years invited, 33% completed testing. FIT positivity rate was 4%, and 85% of those with a positive test underwent colonoscopy.…
