Endovascular treatment (EVT) is an established option for selected patients with large-vessel occlusion (LVO) and acute ischemic stroke. EVT was first established for patients who could be treated within 6 hours after stroke onset. Subsequently, trials using advanced imaging demonstrated benefit in selected patients up to 16 or even 24 hours after stroke onset. Whether even later treatment has benefit has not been studied. These authors retrospectively analyzed data from patients who had ischemic stroke and LVO (of the internal carotid or middle cerebral artery) who presented to one center beyond 16 hours.
