Ablation for paroxysmal atrial fibrillation (AF) involves pulmonary-vein isolation (PVI). However, the optimal technique for persistent AF is not yet known although almost certainly it is not solely PVI. Pulsed field ablation (PFA) is an energy source that selectively ablates myocardial tissue, making it attractive for AF ablation (NEJM JW Cardiol Jul 17 2019; [e-pub] and J Am Acad Cardiol 2019; 74:315).
In an extension of research showing that PFA creates durable PVI lesions, researchers in an industry-sponsored, two-center trial evaluated whether the posterior wall could also be effectively and safely ablated in 25 patients with persistent AF who underwent PFA of the pulmonary veins and posterior wall (NCT04170621).
