Women with body-mass index ≥30 kg/m2 are at greater risk for major postoperative wound complications. Investigators compared two approaches to wound management among obese women undergoing cesarean delivery at four academic and two community hospitals in the U.S. Patients were randomized to negative pressure wound therapy (NPWT; 816) or standard bandage dressing (SBD; 808) using gauze and adhesive tape.
NPWT and SBD yielded similar outcomes (superficial wound infections, 2.1% overall; deep infections, 1.4%; organ space infections, 0.3%). Rates of surgical-site infections in the NPWT and SBD groups were not statistically different (3.6% and 3.4%; relative risk, 1.05; 95% confidence interval, 0.6–1.8).
