Ticagrelor is a reversible platelet P2Y12 receptor inhibitor. The recently completed THALES trial, comparing aspirin plus ticagrelor (A+T) versus aspirin alone in patients with high-risk transient ischemic attack (TIA) or mild stroke, found a 17% reduction in stroke or death within 30 days in the overall trial population. Whether the A+T combination is more effective in certain stroke subtypes has not been explored. In a preplanned exploratory analysis, the authors have now compared treatments effects in THALES trial participants with likely atherosclerotic stroke (≥30% of an intracranial or extracranial artery linked with clinical symptoms) and in participants with stroke of other mechanisms.
