Current guidelines recommend 4 to 6 weeks of antibiotic therapy for diabetic foot osteomyelitis (DFO). However, longer courses (>6 weeks) are frequently given because of concern for poor outcomes, although few published data support prolonged administration of antibiotics.
To investigate antibiotic optimization for DFO, researchers from Switzerland and the U.S. designed a pilot randomized controlled trial that compared 3 weeks versus 6 weeks of systemic antibiotic therapy. The unblinded study of 47 antibiotic regimens was conducted from February 2017 until October 2019 among adults who had undergone initial debridement of necrotic tissue.
