Treatment with endovascular thrombectomy (EVT) for patients with an acute stroke and large-vessel occlusion (LVO) is recommended only for those with “nonminor” deficits and a documented NIH Stroke Scale (NIHSS) score ≥6.
These authors analyzed a multicenter registry of patients with minor stroke (NIHSS score of ≤5) who were treated with alteplase to determine the frequency of early neurologic deterioration (END) — defined as an increase of ≥4 points on the NIHSS within 24 hours after alteplase administration — and factors associated with END. Based on these results, they constructed a score to predict END. They also evaluated outcomes of patients with END who did and did not undergo rescue thrombectomy.
