Adjuvant chemotherapy after resection of pancreatic adenocarcinoma improves survival. Positive results have been reported for postoperative use of gemcitabine, gemcitabine plus capecitabine, and recently FOLFIRINOX, which is the current standard of care (NEJM JW Oncol Hematol Mar 2019 and N Engl J Med 2018; 379:2463). Neoadjuvant chemotherapy is being explored to potentially enhance tolerance and delivery of systemic chemotherapy added to surgery, with hopes of improving rates of resection and survival.
