Edge-to-Edge Repair for Secondary Mitral Regurgitation at 3 Years: Sustained Benefits

A 3-year follow-up of the COAPT trial confirms the initial findings and provides data on patients who crossed over from medical to percutaneous therapy.

The manufacturer-supported, randomized COAPT trial demonstrated the benefit of transcatheter edge-to-edge repair (TEER) with MitraClip at 2 years for patients with heart failure (HF) and secondary mitral regurgitation (MR), compared to guideline-directed medical therapy (GDMT) only. The researchers now report an additional year of follow-up in about 90% of the participants and describe outcomes for patients originally assigned to the GDMT-only group who later received TEER.

The annualized rate for all HF hospitalizations was 69% for GDMT only and 36% for GDMT plus TEER (hazard ratio, 0.49). Death from any cause was 56% and 43%, respectively (HR, 0.67). The numbers needed to treat with TEER to prevent 1 event were 3.0 for hospitalization and …
