Results of the landmark SPRINT study were published in late 2015 (NEJM Gen Med Dec 1 2015 and N Engl J Med 2015; 373:2103). High-risk, middle-aged and older patients with hypertension were randomized to systolic blood pressure (SBP) targets of 120 mm Hg or 140 mm Hg. During average follow-up of 3.3 years, the mean SBPs in the two groups were 122 mm Hg and 135 mm Hg, respectively. The incidence of a composite cardiovascular (CV) endpoint was significantly lower with intensive than with standard treatment (5.2% vs. 6.8%). Hypotension, syncope, electrolyte abnormalities, and decline in glomerular filtration rate (GFR) occurred slightly more often in the intensive-treatment group.
