Variability in the prothrombin time/international normalized ratio (PT/INR) presents challenges for physicians and frustrations for patients anticoagulated with warfarin. Fluctuations in the PT/INR may result from reductions in factor VII, but the antithrombotic effect of warfarin is likely caused by reductions in factors II and X. Fluctuating PT/INR values can lead to changes in safety and efficacy of treatment as well as additional lab testing.
