Some patients with type 2 diabetes find it difficult to manage complex basal-bolus insulin regimens. In this pragmatic, open-label trial from Italy, researchers examined whether giving basal insulin only — in combination with a glucagon-like peptide-1 (GLP-1) agonist or a sodium–glucose cotransporter-2 (SGLT-2) inhibitor — would be comparable to continuing basal-bolus insulin.
The trial involved 305 patients with type 2 diabetes (mean age, 62; mean glycosylated hemoglobin [HbA1c], 8.6%) who already were using basal-bolus insulin with four injections daily. Patients were randomized to the following three groups:
