Chronic midportion Achilles tendinopathy is common in recreational athletes and often is unresponsive to physical therapy. The pursuit of alternative therapeutic approaches has led to use of platelet-rich plasma (PRP) injections, based on a theoretical benefit of tissue repair and growth promoting factors in PRP, but with little clinical evidence (NEJM JW Gen Med Feb 1 2010 and JAMA 2010; 303:144). In this randomized, single-blind trial of intratendinous PRP injection, researchers randomized 240 adults (mean age, 52) with chronic midportion Achilles tendinopathy in whom usual approaches had failed and who had been referred for surgery.
