Alpha-synuclein (α-syn) seeding activity in the cerebrospinal fluid (CSF) via real-time quaking conversion (RT-QuIC) has been associated with Parkinson disease and dementia with Lewy bodies (DLB). In this study, researchers investigated whether the α-syn CSF RT-QuIC assay could identify underlying Lewy body (LB) disease in those with mild cognitive impairment (MCI) from two independent cohorts in Bologna and Amsterdam. Participants were classified as MCI-LB (n=81), MCI-Alzheimer disease (MCI-AD; n=120), unspecified MCI (n=30), or cognitively normal controls (n=58) based on clinical features, AD biomarkers, imaging, and neurophysiologic data. CSF was considered positive if two out of four tests were positive for α-syn seeding activity.
