Primary care physicians commonly refer older patients with unexplained anemia to hematologists, as myelodysplastic syndrome (MDS) is considered prominently in the differential diagnosis. Evaluation for MDS requires a bone marrow biopsy to rule in or exclude the diagnosis. While considered routine, the procedure can cause discomfort, and when objective features such as cytogenetic abnormalities are lacking, diagnosis based on morphology alone can be challenging.
