Finerenone (Kerendia) is a nonsteroidal mineralocorticoid-receptor antagonist; it reportedly has more anti-inflammatory and antifibrotic effects than its steroidal counterparts (e.g., spironolactone, eplerenone). Based on a 2020 randomized trial (NEJM JW Gen Med Dec 1 2020 and N Engl J Med 2021; 383:2219), the FDA approved finerenone in July 2021 to lower risks for adverse renal and cardiovascular outcomes in patients with type 2 diabetes and chronic kidney disease (CKD).
