On October 12, 2021, the U.S. Preventive Services Task Force (USPSTF) released a new draft recommendation statement on use of low-dose aspirin for preventing adverse cardiovascular (CV) events in people without known CV disease. The statement notes that, for middle-aged people (age range, 40–59) with 10-year CV risk ≥10%, “the decision to initiate aspirin … should be an individual one.” The USPSTF gives this recommendation a “C” grade, indicating that net benefit is likely to be small and that decisions to begin aspirin thus should reflect “professional judgment and patient preferences.” But for older people (age, ≥60), the USPSTF recommends against initiating low-dose aspirin for primary prevention.
