The biologic agent adalimumab, a tumor necrosis factor inhibitor, is U.S. FDA–approved for treating patients with moderate-to-severe hidradenitis suppurativa (HS; NEJM JW Gen Med Sep 1 2016). An important question is whether biologic therapy is effective and safe in patients who are undergoing surgery for HS.
In this industry-sponsored study, researchers enrolled 206 patients who required wide-excision surgery for an area of HS in the axillary or inguinal region and who had active HS inflammatory lesions in at least one other site. Patients were randomized to receive adalimumab or placebo starting 12 weeks before planned surgery and continuing for 12 weeks postoperatively.
