In the Diabetes Prevention Program, 3200 adults (mean age, 51) with prediabetes and obesity were randomized to an intensive lifestyle intervention, metformin, or placebo (NEJM JW Gen Med Mar 15 2002 and N Engl J Med 2002; 346:393). In findings published in 2002, participants in the lifestyle and metformin groups were significantly less likely to have met glycemic criteria for diabetes at 3 years. After completion of the randomized trial, most participants continued in a long-term follow-up phase, during which those in the metformin group continued to receive metformin, and those in the lifestyle group were offered additional “reinforcement” sessions.
